Terms & conditions

The first thing we will do is to have an initial consultation to establish how I can help you and for you to ask me any questions.  This conversation will give me the information I need to develop an individualised, safe and effective programme and will cover things like medical background overview, current and future treatments, any side affects you are experiencing, your goals, and current and past exercise etc.  

Before we can start working together, you will need to complete some paperwork, including:

  • PAR-Q form (health questionnaire)
  • Informed Consent form 
  • Sign a Client-Trainer Agreement to say that you are happy with my Terms & Conditions  

All information you give me will be kept in confidence. 


PAR-Q (Pre-Activity Readiness Questionnaire)  

This is an important health screening questionnaire that determines your safety when starting a new exercise programme. It highlights any medical conditions that I need to take into consideration when planning a safe and effective exercise programme. Please disclose all health information as requested in the screening questionnaire and at our initial consultation and keep me updated and informed of any changes to your health status. This includes all medical conditions; physical and mental, injuries, allergies, and medication you are taking. It may be that you need to seek clearance from your healthcare professional prior to us commencing an exercise programme and if so, we will discuss this.
 

Informed Consent 

Whilst for most people exercise is hugely positive with many health benefits, there remains some risk. By signing the informed consent form you declare that you have understood what is involved in our work together and any potential risks.
 

Client – Trainer Agreement

My commitment to you:

  • All information you give me will be kept in confidence. Your personal details and medical information will not be made available to third parties unless permission to do so is granted by you.
  • I intend to work with you within the scope of my knowledge and competencies as a Can Rehab Registered Exercise Professional.​
  • Using my training and experience, I will do my best to deliver a safe and effective exercise programme and offer lifestyle advice within the scope of my practice that I believe will help you achieve your personal goals. ​If I feel that it is in your best interests to be referred to another health professional for more specific advice, I will discuss this with you. ​
  • I will ensure all clients complete a PAR-Q and Informed Consent form before commencing any exercise programme. I may also require a letter of ‘medical clearance’ from your healthcare professional. Please be aware that there could be charge for this.​
  • If I am forced to cancel our session I will reschedule as soon as possible with no charge.
  • If I run late in starting our session, I will extend the session time accordingly or make it up at a future date, whichever is most convenient to you.
  • I will endeavour to be as flexible as I can be to rearrange sessions changed by you with more than 24 hours and within 24 hours for extenuating circumstances or ill health. 

Your responsibilities to me:

  • Complete a PAR-Q form (health questionnaire), Informed Consent form and sign this Client-Trainer Agreement to say that you are happy with my Terms & Conditions
  • Inform me of all relevant medical history and any new medical or physical reason which may impact your participation in a certain exercise or training session. 
  • Let me know if you feel unwell, experience any changes in symptoms or medication.
  • If you or I am unsure if it is safe for you to exercise, agree to get clearance from a healthcare professional to participate in exercise.  
  • Make payment prior to each session via BACS.
  • Contact me as soon as possible if you cannot make a scheduled session.
  • Agree to my cancellation policy of 100% of the session fee being applicable for sessions cancelled within 24 hours (or for non-attendance).
  • Let me know if you have any specific requirements for your exercise programme so I can accommodate them where appropriate. 
  • Be committed to work towards your agreed goals.
  • Keep me fully informed of any issues that arise for you throughout our working relationship.
  • Have a small snack or light meal two or three hours before your session and ensure you are well hydrated. 
  • Bring a water bottle to your session and wear appropriate clothing and footwear (please ask me if you are unsure).

GDPR Policy

You give permission to be contacted by Vicky Pudney by email, phone, text or online messaging in relation to LiveWell Fitness services.

You give permission for Vicky Pudney to contact your GP, doctor, consultant or any other health care professional if required.

Your personal information will be treated with complete confidentiality and will not be shared with anyone else for any reason, apart from referral to a health care professional if required and with your permission.

Your personal details, including medical history and any notes taken by Vicky Pudney will be stored securely (in both hard copy and digitally) for the length of time we are working together and for 6 years thereafter. Online communications will be held on a computer protected by a password.

Your personal details will not be shared with any third party and will not be used for marketing purposes, apart from in relation to services from LiveWell Fitness.

 

Your Legal Rights

Under certain circumstances, you have rights under data protection laws in relation to your personal data. These include the right to:

  • Request access to your personal data.
  • Request correction of your personal data.
  • Request erasure of your personal data.
  • Object to processing of your personal data.
  • Request restriction of processing your personal data.
  • Request transfer of your personal data.
  • Right to withdraw consent.
